Limiting Collection Of Personal Information
It is always your choice to provide us with your personal health information. You may withdraw your consent at any time (subject to legal restrictions and with reasonable notice). You may limit the information that we are able to share, or limit who we share it with. Any limitation put on your personal health information may restrict the services we are able to provide to you. We will do our best to resolve any concerns that you may have so that we can work together to provide you with quality health care.
Limited Use, Disclosure & Retention
We will only use or share the information you give us to reach your health care goals. We are legally required to keep your information for ten (10) years, and for children, ten (10) years after their eighteenth (18th) birthday (Regulated Health Professions Act).
Accuracy
We will keep your personal health information as accurate, complete and up-to-date as possible.
Safeguards:
Our files, computers and any location where health information is stored is kept secure. We ensure that all staff and students have signed a confidentiality agreement and have received training on the organization’s privacy policies. N’Mninoeyaa, Baawaating and all partnering health centres adhere to the government regulations set out in the following Acts:
• Personal Information Protection and Electronic Documents Act (PIPEDA)
• Personal Health Information Protection Act (PHIPA)
In the unlikely event that your personal health information is stolen, lost or accessed by unauthorized persons, you will be notified immediately.
As a health services organization, we review our privacy policies and procedures annually to ensure all privacy, security and confidentiality practices are adhered to. We also closely monitor our electronic medical records (EMR) by running regular audits to ensure your health information is safe.
